Coming out of Hospital
Deciding to care or continue caring for someone who is coming out of hospital and who can no longer care for themselves in the same way as before can be difficult.
This factsheet explains your rights as a carer during the hospital discharge procedure, the steps that should be followed before the person you care for is discharged from hospital, what your rights are, and what to do if things go wrong.
These factsheets that are available in Welsh and English applies to people living in Wales. They take into account important new rights for carers that were introduced in April 2016 in the Social Services and Well-being (Wales) Act.
