Cwm Taf University Health Board’

Together for health heart delivery plan

Together for Health – a Heart Disease Delivery Plan” was published by the Welsh Government in 2013 and provides a framework for action by Local Health Boards and NHS Trusts working together with their partners.

It sets out the Welsh Government’s expectations of the NHS in Wales to prevent avoidable heart disease and plan, secure and deliver high quality person-centred care for anyone affected by heart disease.

It focuses on meeting population need, tackling variation in access to services and reducing inequalities in health outcomes across the following 6 themes:
Promotion of healthy hearts
Timely detection of heart disease
Fast and effective care
Living with heart disease
Improving Information
Targeting research

The plan sets out how Cwm Taf Local Health Board (CTLHB), will address the key priorities set out by Welsh Government.
